BCSSA Accident and Incident Policy - Incident Steps to Follow

  1. If an incident occurs, attend to those involved to ensure there are no injuries.

  1. Complete a BCSSA Incident Report Form. The BCSSA Incident Report Form must include at least.

  • names and contact information of the reporter and witnesses;

  • a detailed description of the incident; and

  • Recommendations to prevent similar incidents in the future.

  • *Note- If possible, take pictures of the area where the accident.

  1. Contact BCSSA Provincial Office for advice on whether an incident should be reported to the insurance company. Incidents are often reported even if a claim may not come out of them.

  1. Document the incident details on the BCSSA Incident Report Form, gather statements from all witnesses and complete the BCSSA Incident Report Form.

  1. Keep the BCSSA Incident Report Form on file:

  • Documentation should be kept for as long as necessary to defend against potential claims.  The length of time that legal action may be taken by law is called the Statute of Limitations.

  • If the individual involved is an adult, keep the BCSSA Incident Report Form documentation on file for at least three to five years after the Statute of Limitations passes for that type of claim.            

  • If the individual involved is a minor, keep the BCSSA Incident Report Form documentation on file for at least three to five years after the Statute of Limitations passes for that type of claim once the minor has reached the age of majority (19 years in BC).

  • An efficient method of organizing and storing incident reports is in an electronic database where they can be sorted and retrieved based on a number of factors. BCSSA will assign an incident number to each report received.

  1. Review the BCSSA Incident Report Form to determine the cause of the incident and any preventative measures that could be implemented.

  1. Submit copies of the BCSSA Incident Report Form to the Regional Director, Club President and BCSSA Provincial Office within five (5) business days.
