BCSSA Accident and Incident Policy - What Warrants Documentation

A general rule of thumb for documentation of incidents or accidents is...if you are unsure...fill out the form!

bullet

The forms are completed to inform the BCSSA Provincial Office (as well as local and regional reps) that an incident/accident has occurred that could have serious ramifications.

bullet

The form should be filled out as soon as possible after the incident/accident.

 

Copies must be sent to the Club President, Regional Director, and to the BCSSA Provincial Office within 5 working days.
